A. Your health plan is committed to helping you achieve your best health. Incentives for participating in a wellness program are available to all employees on the health insurance plan. If you think you might be unable to meet a standard for an incentive under this wellness program, you might qualify for an opportunity to earn the same incentive by a different means. Contact BTD HR Department and we will work with you (and, if you wish, with your doctor) to find a wellness program that is right for you in light of your health status that may qualify you to earn the incentive. The single greatest opportunity to improve health lies in personal behavior. BTD would like to build a culture of health to control costs; increase productivity; reduce absenteeism; attract and retain employees; and most importantly, improve the health of your personal wellbeing. BTD will continue with the glucose testing to help you with identifying potential risks and/or early detection of diabetes. BTD will use the general data collected from these tests to evaluate for future years and to help determine if a discount will be offered. Please note BTD picked up the costs for the glucose testing.

B. BTD will continue to offer a $2.50 weekly discount for you and your spouse per screen per person for Body Mass Index (BMI), Blood Pressure, and LDL Cholesterol. The Nicotine discount is $7.50 per person per week for you and your spouse. Each incentive that is met will be a decrease to your weekly employee contributions for a total possible savings of $15 for employee and an additional $15 for spouse.

iv. Nicotine/Tobacco Negative

C. To receive the incentive, employees and their spouses must participate in a screening at the clinic or at their doctor’s office before 11/30/25. Any screenings completed after this time will not be started until the following month. Nicotine incentive screens may be scheduled by setting up an appointment with HR. The clinic will notify Batesville Tool & Die, Inc. who qualifies for which incentives by submitting the completed incentive form to HR. If you choose to go to your own doctor, please pick up a form from HR and take with you. The doctor’s office will fill in the information, and you can bring it back to HR or have the doctor fax it.

D. The weekly contributions will be adjusted beginning the 2nd Friday in December.
