Wellness Program

Who is Eligible and When: All employees of Pathfinder Services can participate in the wellness panel screening at no cost. Employees enrolled in the 2026 medical plan are eligible to earn premium discounts applicable to the 2026 medical plan year.

The program is strictly voluntary. Employees are under no obligation to participate.

What You Must Do: To be eligible for the Pathfinder Services Premium Incentive Program, you must complete a health screening through the Onsite Health Diagnostic Screenings and meet certain health targets. If your health screening results indicate an elevated level of risk, you will have the option to complete another reasonable alternative standard for the chance to earn that incent

Benefits You Receive: If you have satisfied all elements of the plan, you will be eligible for incentives based on the results of your screening.

All employees will have access to our wellness portal called Personify! You can register your account by going to: .Log in to Personify Health or download the app on your phone - just search Personify Health and download.

From there you'll want to enter the sponsor Name: Unified Group Services and complete the demographic information to confirm eligibility. By completing various activities and logging them within the portal, you can earn points!

Your health plan is committed to helping you achieve your best health. Rewards for participating in the wellness program are available to all employees. If you think you might be unable to meet a standard for the reward under the wellness program, you might qualify for an opportunity to earn the same reward by different means. Contact Human Resources at 260-355-2571 and we will work with you (and if you wish, your doctor) to find a wellness program with the same reward that is right for you in light of your health.
