A GROWING DIABETES BURDEN
American Senior Communities operates in an environment with high employee turnover and increasing health risk, which intensified chronic disease management challenges.
THE CATALYST
The Question That Changed Everything
“Are we spending this much more on diabetes—and our population is getting sicker?”
This question from our team and the American Senior Communities board became a defining moment. It exposed a critical disconnect: rising healthcare costs without meaningful improvement in patient outcomes.
Despite increased investment in medications and treatment, there was no clear evidence that members were getting healthier. This realization prompted a shift—from reactive care to a more accountable, outcomes-driven model focused on both cost and clinical impact.
Diabetic Rx spend $2.8M in 2023 to $3.9M 2024 (39% increase)
27% of the total cost of health care was going to diabetes care representing 10% of the population. This population was still sick and at risk

annual increase in diabetes-related care costs
of members with diabetes accessed prescription medications
of pharmacy spend tied to diabetes medications
of total prescriptions related to diabetes management
ADDITIONAL CHALLENGES

Rising GLP-1 medication costs

Gaps in preventive and chronic care

High rates of uncontrolled A1c

Fragmented medication management

Without intervention, these trends would continue to increase medical costs and worsen member outcomes.
